About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. You will work with physicians and clinicians to manage care plans that support recovery and help patients regain independence. The role is patient-facing and requires participation in Humana’s TB screening program. It is also subject to Humana’s driver safety program, including requirements to maintain a valid state driver’s license and personal vehicle liability insurance. The position is scheduled for 40 hours per week.

Requirements

Current nursing license in the practicing state.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Two years of experience as an LPN/LVN in a clinical setting.
Must complete TB screening if selected.
Must maintain personal vehicle liability insurance at the applicable state minimum limits or the higher limits specified in the posting.

Nice To Haves

Nursing experience in a Home Health or Hospice setting.

Responsibilities

Provide skilled nursing care to patients in their homes under the direction of an RN.
Implement individualized care plans in collaboration with the patient, family, and healthcare team.
Educate patients and their families on disease management, treatment options, and self-care techniques.
Maintain accurate records of patient care, including visit documentation, interventions, and outcomes.
Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
Participate in care coordination activities and discharge planning.
