About The Position

The Home Health/Hospice Case Manager role is a full-time position requiring an experienced RN or LPN. This role involves coordinating client care, acting as a point of contact for all disciplines, and overseeing visit frequency. The Case Manager will consult with physicians and the office regarding patient care, perform accurate OASIS collection, maintain current medication profiles, ensure timely reporting of lab values, and participate in case conferences and meetings. Providing patient care with respect for rights, dignity, and needs, assessing physical, psychological, and social needs, managing pain, and documenting according to guidelines are essential duties. Reviewing patient history, diagnostics, and lab data, and reporting abnormalities are also key responsibilities.

Requirements

  • Current, unencumbered RN or LPN license
  • Certification in CPR
  • Prior clinical experience
  • Excellent communication skills
  • Demonstrated ability to maintain a high level of professionalism during stressful times
  • Strong critical thinking skills
  • Ability to adapt to different clinical settings and environments

Responsibilities

  • Perform visits and complete coordination of client care.
  • Act as the point of contact for all disciplines involved in patient care.
  • Oversee the frequency of visits for the episode of care.
  • Consult with the physician and office regarding patient care.
  • Perform accurate OASIS collection.
  • Ensure the medication profile remains current.
  • Ensure lab values have been reported to the physician timely.
  • Attend weekly case conference and monthly case manager meetings.
  • Provide all patient care in a manner that reflects respect for patient rights, dignity, and expressed needs.
  • Assess the patient’s physical, psychological, and social needs.
  • Assess pain and implement appropriate pain management techniques.
  • Document patient history, symptoms, assessments, tests, and care according to guidelines.
  • Review patient history, diagnostics, and lab data, and report any abnormalities.
