About The Position

Avita Health System is seeking a dedicated Registered Nurse (RN) Case Manager to join their Home Health & Hospice Department in Galion, Ohio. The role involves overall care coordination for home health or hospice patients from referral to discharge, ensuring timely and appropriate service delivery that promotes quality and cost-effective outcomes. The RN will work closely with visiting nursing staff, provide direct patient care, and act as a patient advocate and primary contact for patients, caregivers, family, insurance companies, doctors, and staff to ensure effective communication regarding patient care.

Requirements

Associate’s Degree (A.D.), Diploma, Bachelor’s Degree (B.S.), or Master of Science (M.S). from an approved school of nursing.
Minimum of two (2) years’ previous home health or hospice experience or in combination with supervisory experience.
Current State of Ohio Registered Nurse licensure.
BLS certification or obtained within 30 days of hire.
Current valid Ohio Driver’s license.

Responsibilities

Overall care coordination of each home health or hospice patient from the time of referral to the time of discharge.
Ensures the timely delivery of appropriate service, which promotes quality and cost-effective health care outcomes, working within established agency policies and Home Health and Hospice regulations.
Works closely with visiting nursing staff, providing direct patient care to ensure that care is goal-directed and patients' needs are being met.
Acts as a patient advocate and primary contact for patients, caregivers, family, insurance companies, doctors, and staff to ensure effective communication regarding patient care.
