About The Position

Enrich the lives of older adults through community, choice, and continuing care in the Home Health & Hospice Aide role. The Home Health & Hospice Aide provides individualized, compassionate personal care and supportive services to patients in their homes under the supervision of a Registered Nurse or licensed clinical professional and in accordance with the Plan of Care. This role supports patients with activities of daily living (ADLs), comfort measures, and safety needs to promote dignity, quality of life, and independence during home health or end-of-life care. Requirements

  • Driving is an essential function and/or a legitimate business purpose of this position; therefore, a valid driver’s license is required as a condition of employment.
  • Active Nursing Assistant certification in Washington State
  • CPR and First Aid certification within 30 days of hire
  • Ability to read, write, and communicate effectively in English.
  • Physical ability to perform job duties, including lifting, bending, standing, and transferring patients.

Nice To Haves

  • Prior experience in home health, hospice, long-term care, or assisted living
  • Prior experience with end-of-life care and comfort-focused services

Responsibilities

  • Assist patients with activities of daily living (ADLs) including bathing, grooming, toileting, dressing, and mobility.
  • Provide comfort-focused care for hospice patients, including positioning, skin integrity support, and personal hygiene.
  • Assist with safe transfers and ambulation using appropriate equipment and techniques.
  • Perform basic household tasks related to patient care (e.g., light meal preparation, tidying patient areas, laundering patient linens).
  • Support patients’ emotional, psychological, and spiritual comfort in alignment with hospice philosophy.
  • Encourage patient independence and respect patient rights, preferences, and cultural values.
  • Measure and record vital signs, intake/output, and other observations as delegated and trained.
  • Observe, document, and report changes in patient condition to the supervising nurse promptly.
  • Always follow the patient’s individualized care plan and agency policies.
  • Assist with nurse-delegated tasks when authorized and trained under Washington State Nurse Delegation laws.
  • Complete accurate and timely documentation of care provided per agency standards.
  • Communicate effectively with patients, families, nurses, and interdisciplinary team members.
  • Participate in care team meetings, in-services, and required training.
  • Maintain strict adherence to infection control and safety procedures.
  • Follow all applicable company, federal, state, and agency regulations.
  • Maintain patient confidentiality in accordance with HIPAA regulations.
