Requirements
- Current RN License
- Valid Driver’s License
- Current CPR Certification
- Valid Auto Insurance
Nice To Haves
- 2+ years of Home Health RN experience
- HomeCare HomeBase experience
Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing care and ongoing patient evaluations
- Educate patients and families on medications, disease management, nutrition, and self-care
- Collaborate with physicians and interdisciplinary teams to ensure quality outcomes
- Monitor patient progress and communicate changes in condition promptly
- Supervise Home Health Aides and LPN/LVNs
- Maintain accurate and timely clinical documentation
- Deliver compassionate, high-quality care that helps patients remain safe and independent at home
