About The Position
The Home Health Field LVN/LPN delivers nursing care to patients in their homes, supporting comfort, safety, and the management of health conditions. The role involves assessing patient health status, administering medications, providing wound care, and collaborating with physicians, therapists, family members, and other care-team members. Under the direction of an RN, the LVN assists with identifying patient needs and participates in planning and implementing care in accordance with the plan of care. The position also involves educating patients and caregivers on treatment and health maintenance to promote independence, recovery, and chronic disease management.
Requirements
- Current LVN license.
- Valid driver’s license.
- Valid auto insurance.
- CPR certification.

Responsibilities
- Under the direction of the RN, assist in identifying patients’ physical, psychosocial, and environmental needs, using documentation, clinical records, team reports, and evaluations.
- Participate in planning and implementing care with the RN in accordance with the plan of care (POC).
- Apply infection-control concepts and standard precautions when coordinating and performing patient-care activities.
- Assure continuity of care through delivery of quality patient care.
- Communicate effectively with patients and families, team members, and other health care professionals, as reflected in clinical notes, case conferences, communication notes, and evaluations.
- Monitor assigned cases to ensure compliance with third-party payer requirements.
- Promote the agency’s philosophy and administrative policies to ensure quality of care.
- Assess patient health status, administer medications, and provide wound care.
- Educate patients and caregivers on treatment protocols and health maintenance.
