Requirements
- Current, unrestricted LVN/LPN license in the state of practice
- High school diploma or equivalent
- Current BLS/CPR Certification
- Minimum 1 year of nursing experience
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout the assigned service area
Nice To Haves
- Home Health experience preferred
- Home Health, Hospice, or Community Health experience
- Knowledge of Medicare Home Health regulations a plus
- Experience using HomeCare HomeBase or other EMR systems
- Strong organizational and time-management skills
Responsibilities
- Perform ongoing patient assessments and report changes in condition to the supervising RN and physician.
- Administer medications, injections, and prescribed treatments safely and accurately.
- Provide wound care, catheter care, and other skilled nursing procedures.
- Educate patients and caregivers on medications, disease management, safety, and home care techniques.
- Monitor patient progress and communicate updates with the care team.
- Document all patient visits accurately and timely using the electronic medical record (EMR).
- Ensure compliance with Medicare, agency policies, and infection control standards.
- Deliver compassionate, patient-centered care while maintaining patient dignity and confidentiality.
