About The Position
Choice Health at Home is looking for a compassionate and skilled Licensed Vocational Nurse (LVN/LPN) to join our growing Home Health team. As a Home Health LVN, you'll help patients recover, manage chronic conditions, and maintain their independence—all from the comfort of their own homes. You'll work alongside experienced clinicians in a supportive, patient-first environment where your care truly makes an impact.
Requirements
- Current LVN/LPN license
- Valid driver's license and reliable transportation with current auto insurance
- Current CPR certification
Nice To Haves
- Home health experience preferred
- HomeCare HomeBase experience is a plus
- Excellent communication and organizational skills
- A compassionate, patient-centered approach to care
Responsibilities
- Deliver skilled nursing care in patients' homes under the supervision of a Registered Nurse
- Assist with patient assessments and contribute to individualized care plans
- Administer medications and provide treatments as ordered
- Perform wound care and monitor patient progress
- Educate patients and caregivers on disease management, medications, and home care
- Maintain accurate clinical documentation and communicate effectively with the interdisciplinary care team
- Follow infection prevention and safety protocols
- Ensure high-quality, compassionate care while promoting patient independence
