About The Position

Signature Healthcare at Home is seeking to add a COTA for Whatcom County! This position would cover Whatcom County (Bellingham, Lynden, Blaine, Ferndale, Sudden Valley, Deming) for home health occupational therapy services. This position works Monday through Friday day shift, with self-directed scheduling dependent on patient needs and territory. Requirements

  • Must have graduated from an OT assistant program accredited by the ACOTE of the AOTA or any successor organization of the ACOTE.
  • Must be eligible for or have taken the OT assistant certification examination administered by the NBCOT.
  • Must be licensed and able to practice in Washington State.
  • The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job.
  • Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.
  • Possesses and maintains a valid CPR certification.

Nice To Haves

  • Clinicians with experience in Home Health or post-acute care.
  • Familiarity with Homecare Homebase EMR.

Responsibilities

  • Participates in the on-going evaluation of the patient's functional status (muscle function, endurance, visual coordination, written/verbal communication skills, self-care ability, work capacity, etc.) as delegated by the registered Occupational Therapist.
  • Teaches patient and/or family/caregivers new skills and retrains once familiar with daily activities that has been lost to illness or injury.
  • Maintains appropriate clinical records, clinician notes, and reports to the Occupational Therapist any changes in the patient's condition.
  • Evaluates home environment for hazards or barriers to more independent living as deleted by the Occupational Therapist.
  • Follows treatment program and goals for improved patient function as established and documents patient's response to treatment plan and progress towards goals.
  • Identifies patient and family/caregiver needs for other home health care services and consults with interdisciplinary team to get the necessary referrals, information, etc. as needed or appropriate.
