About The Position

Elara Caring is excited to offer a unique opportunity for a Full-Time Home Health CNA to join our Shelton Branch Skilled Home Health Department. This position provides the opportunity to work closely with a skilled interdisciplinary team while supporting patients with medical needs in the comfort of their homes. The CNA will provide services throughout the branch’s entire coverage area and will collaborate closely with Registered Nurses, LPNs, Physical Therapists, Occupational Therapists, Speech Therapists, and other members of the interdisciplinary team. Service area includes to travel throughout the Shelton Branch coverage area below: Seymour, Ansonia, Derby, Shelton, Monroe, Trumbull, Stratford, Bridgeport, Easton, Fairfield, Weston, Westport, Wilton, Norwalk, New Canaan, Darien, Stamford, and Greenwich. Reliable transportation is required. Ability and willingness to travel throughout the full Shelton Branch service area.

Requirements

Current Certified Nursing Assistant (CNA) license in the state of employment
A compassionate, patient-focused approach to care
Excellent communication and interpersonal skills
Ability to lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment
Reliable transportation to travel to patient homes

Responsibilities

Supporting patients with medical needs in the comfort of their homes.
Providing services throughout the branch’s entire coverage area.
Collaborating closely with Registered Nurses, LPNs, Physical Therapists, Occupational Therapists, Speech Therapists, and other members of the interdisciplinary team.
Traveling throughout the full Shelton Branch service area.
