About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the Enola, PA area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team. The CNA will provide hands-on care in the home, assisting with activities of daily living (ADLs), observing and reporting changes in condition, and educating patients and caregivers. The position requires prompt response to patient needs, effective communication with the care team, and accurate documentation in the EMR. The role is independent but with strong team support, requiring travel to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Valid driver’s license
• Ability to spend approximately 20% of your workday driving to and from patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
