About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the Thomasville, PA area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. You will provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence. Additionally, you will educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks. Responsibilities include responding promptly to patient needs, communicating effectively with the care team, and maintaining accurate, timely documentation in the EMR. This is an independent role, but you will not be alone, and will involve travel to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
