About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the Lancaster, PA area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. You will provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence. Additionally, you will educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks. The role requires prompt response to patient needs, effective communication with the care team, and accurate, timely documentation in the EMR. You will work independently, but never alone, and travel to patient homes within an assigned territory.

Requirements

  • Active Certified Nursing Assistant (CNA) certificate in the state you work
  • High School Diploma or certificate
  • Minimum one year healthcare experience
  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
  • Legally authorized to work in the United States
