Requirements

• Valid nursing license (RN or LPN) in the state of Michigan.
• Experience in home health care preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and prioritize patient care effectively.
• Reliable transportation to travel to patient homes in Southfield, MI.
• CPR and Basic Life Support (BLS) certification.

Responsibilities

• Conduct patient assessments to identify healthcare needs and develop individualized care plans.
• Provide skilled nursing care, including wound care, medication management, and chronic disease management.
• Collaborate with physicians, therapists, and other healthcare team members to coordinate patient care.
• Educate patients and their families on treatment plans, medication administration, and health maintenance.
• Document patient progress and maintain accurate medical records in compliance with regulatory standards.
• Respond promptly to patient and family inquiries and adjust care plans as needed.
