Requirements

  • Active Connecticut RN license (unrestricted)
  • Strong Home Health experience required – you know OASIS, workflows, and field realities
  • Leadership experience (or ready to step into leadership from a senior/mentor RN role)
  • Ability to manage both clinical quality and operational priorities
  • Associate degree in Nursing (BSN preferred)
  • Minimum 2 years of Home Health RN experience
  • 1+ year of leadership experience preferred
  • Reliable transportation, valid driver’s license, and auto insurance
  • Willingness to travel within the Middlebury/Waterbury coverage area
  • Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

Nice To Haves

  • Experience with Homecare Homebase or similar EMR is a plus

Responsibilities

  • Lead and support a team of field clinicians delivering skilled care throughout the local community
  • Oversee patient care plans to ensure quality, compliance, and strong outcomes
  • Review OASIS documentation, physician orders, and plans of care
  • Coordinate admissions, case assignments, and timely starts of care
  • Serve as a clinical resource and mentor to your team
  • Partner with physicians, referral sources, and interdisciplinary teams across the area
  • Monitor performance metrics and help drive branch success
  • Assist with hiring and onboarding new clinicians
  • Provide field support as needed
