We’re looking for a Registered Nurse (RN) Clinical Team Manager who thrives on guiding teams, building strong clinician partnerships, and ensuring exceptional patient care. In this role, you’ll lead, mentor, and support a dedicated group of field clinicians—making a meaningful impact on both patient outcomes and team success every day.

Requirements

  • Active Registered Nurse (RN) license (state required)
  • 2–3 years of clinical nursing experience (home health preferred)
  • Prior leadership or supervisory experience strongly preferred
  • Strong communication, organization, and problem-solving skills
  • Passion for mentoring clinicians and improving patient outcomes
  • Ability to manage multiple priorities in a fast-paced environment
  • Reliable transportation for occasional field support

Responsibilities

  • Lead, coach, and support a team of field clinicians (RNs, LPNs/LVNs)
  • Foster a positive, accountable, and patient-centered team culture
  • Oversee onboarding, training, and ongoing clinical development
  • Ensure timely, accurate, and compliant clinical documentation
  • Partner with physicians and interdisciplinary teams to coordinate care
  • Review plans of care, orders, and clinical documentation for quality and compliance
  • Monitor productivity, scheduling, and utilization to support team success
  • Address and resolve clinical, scheduling, and payroll-related issues
  • Support quality assurance, audits, and regulatory compliance
  • Investigate and resolve patient concerns, incidents, and infection control matters
  • Assist with on-call support and step in for patient visits when needed
