Clinical Team Lead (RN) Home Health
About The Position
Aspire Home Healthcare is a growing, multi-state home health organization committed to delivering high-quality, patient-centered care. Our San Diego office is expanding, and we are building a strong clinical leadership team to support that growth. We are seeking a Clinical Team Lead (RN) who brings both clinical expertise and operational ownership to drive outcomes, support clinicians, and ensure excellence in patient care. The Clinical Team Lead works directly with our Regional Clinical Director, to support clinical operations, maintain compliance, and elevate team performance. This is a hands-on leadership role—ideal for an experienced home health RN who is comfortable managing documentation quality, supporting field staff, and ensuring the office is always survey-ready.
Requirements
- Active Registered Nurse (RN) license in California
- Home Health experience required
- Strong knowledge of OASIS, Medicare Conditions of Participation, and compliance
- Experience with EMR systems (WellSky preferred)
- Strong communication, organizational, and problem-solving skills
Nice To Haves
- Prior leadership or team coordination experience preferred
Responsibilities
- Support daily clinical operations across the San Diego office
- Review and ensure accuracy of OASIS, plans of care, and documentation
- Maintain compliance with Medicare, state, and ACHC standards
- Provide real-time clinical guidance to field nurses and therapists
- Participate in case conferences and interdisciplinary team meetings
- Support onboarding, training, and performance of clinical staff
- Drive timely documentation (24-hour expectation) and accountability
- Identify and resolve clinical and documentation issues proactively
- Assist with survey readiness, audits, and quality improvement initiatives
