The Home Health Clinical Manager (RN) oversees appropriate patient and personnel assignments, coordinates patient care and referrals, and assures that patient needs are continually assessed and that individualized plans of care are developed, implemented, and updated. This role provides data to the Executive Director regarding patients and health services, and develops, evaluates, and revises policies, procedures, goals, objectives, and standards of care. The manager also adjusts staffing levels, conducts meetings, and reviews patient care reports. They support a culture of safety and quality practices, coordinate patient services, and aid in retaining quality staff through hiring, orienting, training, evaluating, and disciplining staff. Additionally, they schedule staff for proper coverage, coordinate orientation and training, conduct performance evaluations, and communicate information to the Administrator. The role requires maintaining knowledge of Medicare, Medicaid, and third-party payer requirements, communicating and coordinating with various healthcare professionals and stakeholders, staying updated on new methods and trends, and participating in information management and Quality Assurance Performance Improvement (QAPI) programs. The manager must understand applicable state and federal regulations for home health and the OASIS data collection and reporting process, and provide direct patient care as needed.
