The Home Health Certified Nursing Assistant (CNA) provides care ordered by the physician and directed by the registered nurse or appropriate skilled professional as indicated by the plan of care to all patients on caseload. This includes baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing patients, feeding, and transferring patients. The CNA takes accurate vital signs, demonstrates competency in required skills, and describes the activities of daily living, assisting the patient to meet these activities. They participate in case conferences, notify appropriate licensed staff when a patient complains of pain, and recognize and report changes in skin condition. The CNA positions patients in bed with proper body alignment, assists with ambulation and transfer techniques, and performs patient care in an environment that optimizes patient safety and reduces medical errors. They support cultural practices that do not harm the patient/family or interfere with treatment, identify and communicate findings to the Case Manager and/or Clinical Manager, and participate in other mandated activities as requested. Clear, concise documentation in the medical record is required, relating to reimbursement guidelines and Agency policy and procedure.
