Home Health Bilingual Licensed Vocational Nurse-Per Diem
About The Position
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence in their homes. You work with physicians and clinicians to manage care plans that support recovery. The Home Health LVN provides skilled nursing care in patients’ homes under the direction of an RN and collaborates with patients, families, and the healthcare team to implement individualized care plans. The position is per diem, with pay per visit/unit of $33.00–$47.00. Scheduled weekly hours are 1. The role is patient-facing and requires TB screening. It is also subject to Humana’s driver safety program and requires a valid state driver’s license and personal vehicle liability insurance meeting the applicable state minimums or the stated higher limits. Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including visit documentation, interventions, and outcomes.
- Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
- Participate in care coordination activities and discharge planning.
