They are seeking a compassionate, organized, and team-oriented individual to join their Home Health or team as a Full-Time or Part-Time (Per Diem) Home Health Aide (HHA) in San Francisco, CA. As a Home Health Aide (HHA), you will have the opportunity to collaborate with a dedicated team of healthcare professionals and make a meaningful difference in the lives of patients. The position involves evaluating patients’ physical health, developing individualized treatment plans, and working to improve their mobility, strength, and overall quality of life. You will collaborate closely with the Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.

Requirements

Home Health Aide (HHA) License to practice in California - Required
Experience in Home Health or Hospice care - 1 Year Required
Current CA Driver’s License.
Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

Delivers personalized care and hygiene support to patients in the comfort of their own homes.
Focuses on the promotion of health, prevention of disease and care of the ill at home.
Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
Comply with all company policies and procedures to ensure high standards of care.
