About The Position
ABL Health Care is seeking a compassionate, organized, and team-oriented individual to join their Home Health team as a Part Time (Per Diem) Home Health Aide (HHA) in Brentwood, Antioch, Oakley, and Discovery Bay. As an Home Health Aide (HHA), you will have the opportunity to work alongside a dedicated team of healthcare professionals and make a meaningful difference in the lives of patients. The position involves evaluating patients' health, creating and implementing care plans, and tracking their progress. The role requires working closely with the Clinical Director and Clinical Staff, providing support both remotely and independently in the field.
Requirements
- Home Health Aide (HHA) License to practice in California - Required
- Experience in Home Health or Hospice care - 1 Year Required
- Current CA Driver’s License.
- Able to travel within a 15 - 25 mile radius on a regular basis.
Responsibilities
- Evaluate patients' health.
- Create and implement care plans.
- Track patient progress.
- Provide support both remotely and independently in the field.
- Communicate with patients and clinical staff.
- Call in lab results.
