The Home Health Aide provides care and comfort measures to patients in their homes as directed by care plan, interdisciplinary plan of care, and the provider's medical plan of treatment. Home Health. Part Time position. Lexington region. Provides specific aspects of care under the direction of the clinical leader. Reviews care plan and records visit notes. Documents care as well as significant findings. Reports concerns or changes to leadership. Assists patients with promoting functional ability and maintaining a comfortable home environment. Follows the established plan of care at all times. Assists with or provides basic personal care and hygiene to patients. Provides bedside care in a variety of settings in accordance to Home Care/Hospice policies and procedures and in accordance to the Aide care plan. Assists patients with daily living, personal hygiene (including baths in tubs, shower, basin, bed, shampoos, nail care as prescribed on Aide care plan), ambulation and prescribed exercise. Assists patient with toileting, including use of bedpans and bedside commodes, and provides proper skin care to incontinent patients. Assists with oral or topical medications that the patient can normally self-administer as directed by the patient’s careplan. Measures fluid intake and output as indicated in the care plan. Reviews care plan and asks leaders any questions prior to service. Records visit notes. Reports any changes in the patient’s physical condition, behavior, or appearance to the care team.
