About The Position

The Home Health Aide provides care and comfort measures to patients in their homes as directed by care plan, interdisciplinary plan of care, and the provider's medical plan of treatment. This Part Time position in the Lexington region provides specific aspects of care under the direction of the clinical leader. The role involves reviewing the care plan, recording visit notes, documenting care and significant findings, and reporting concerns or changes to leadership. The Home Health Aide assists patients with promoting functional ability and maintaining a comfortable home environment, following the established plan of care at all times. Responsibilities include assisting with or providing basic personal care and hygiene, bedside care in various settings according to policies and procedures, assisting patients with daily living, personal hygiene (including baths, shampoos, nail care), ambulation, and prescribed exercises. The role also involves assisting patients with toileting, providing proper skin care to incontinent patients, and assisting with oral or topical medications that the patient can normally self-administer as directed by the care plan. Measuring fluid intake and output as indicated in the care plan, reviewing the care plan, asking leaders questions prior to service, and reporting any changes in the patient’s physical condition, behavior, or appearance to the care team are also key duties.

Requirements

H.S diploma or GED
Must have worked as an aide (for compensation) within the 24 months prior to hire or have completed another program as defined by the CMS Home Health Conditions of Participation.
Certified as a nurse's aide and registered by the VA Department of Health Professions.
Satisfactory completion of Geriatric Nursing Assistant, Certified Nursing Assistant or Home Health Aide course that meets State, Federal, and CMS requirements for Home Health Aide training or is deemed competent by the home health agency as required by CMS regulations.
Successfully completes HHA Competence Program as established by home health or hospice agency.
Must have valid driver's license.

Assists patients with daily living, personal hygiene (including baths in tubs, shower, basin, bed, shampoos, nail care as prescribed on Aide care plan), ambulation and prescribed exercise.
Assists patient with toileting, including use of bedpans and bedside commodes, and provides proper skin care to incontinent patients.
Assists with oral or topical medications that the patient can normally self-administer as directed by the patient’s careplan.
Measures fluid intake and output as indicated in the care plan.
Reviews care plan and asks leaders any questions prior to service.
Records visit notes.
Reports any changes in the patient’s physical condition, behavior, or appearance to the care team.
