About The Position

The Home Health Aide plays an essential role in providing compassionate and personalized care to individuals who require assistance with daily living activities in their own homes. This position ensures the safety, comfort, and well-being of patients by delivering high-quality support tailored to their unique health needs. The Home Health Aide collaborates closely with healthcare professionals and family members to implement care plans effectively and monitor patient progress. Requirements

  • High school diploma or equivalent.
  • Completion of a state-approved Home Health Aide training program.
  • Certification as a Home Health Aide (HHA) or equivalent credential as required by the state.
  • Basic knowledge of patient care techniques and safety protocols.
  • Ability to pass background checks and meet health screening requirements.

Nice To Haves

  • Experience in assisted living or long-term care settings.
  • CPR and First Aid certification.
  • Familiarity with electronic health records (EHR) and computer data entry.
  • Strong interpersonal and communication skills.
  • Additional training in specialized care areas such as dementia or chronic disease management.

Responsibilities

  • Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support.
  • Monitor and record patients’ vital signs and health status, reporting any changes to nursing staff or supervisors promptly.
  • Provide medication reminders and assist with prescribed treatments as directed by healthcare professionals.
  • Maintain a clean, safe, and sanitary environment in the patient’s home to prevent infections and promote health.
  • Support patients with meal preparation, feeding assistance, and ensuring proper nutrition and hydration.
  • Document care provided and update patient records accurately using computer data entry systems.
  • Communicate effectively with patients, families, and healthcare teams to coordinate care and address patient needs.
  • Prioritize tasks efficiently to manage time and resources while responding to urgent patient needs.
