About The Position
Provides basic patient care under the direction of a licensed nurse. Serves as patient’s 'Personal Care Champion’ by assisting them with activities of daily living. Provides care & support in the home setting or hospice house with special attention to patient personal care needs in end-of-life care. Perform visits and/or shift work.
Requirements
- Must be able to work independently, have good time management skills, and timely documentation.
- Current Driver's license and proof of insurance for staff performing visits.

- Maintains valid licensure/certification/registration if required
