About The Position

This role focuses on providing one-to-one patient care as a Certified Home Health Aide (HHA). The position emphasizes a whole-person approach, nurturing the mind, body, and spirit of patients. HHAs will follow detailed plans of care, ensuring great health is more than just monitoring vitals. Requirements

Minimum 1 year of paid home-health or hospice experience.
Valid driver’s license and reliable transportation to reach clients’ homes.
CPR certification (or willingness to obtain within 30 days).
Clear, confident communication skills.
A passion for relationship-centered care.

Responsibilities

Provide hands-on ADL support, including safe assistance with transfers, bathing, grooming, toileting, and mobility, while protecting patient dignity.
Maintain a clean and safe home environment by changing linens, doing light laundry, and ensuring the space is hazard-free and sanitized.
Monitor medications and symptoms by providing reminders, observing for changes, and documenting vital signs or concerning signs for the supervising nurse.
Engage in therapeutic activities such as reading aloud, reminiscing, guiding range-of-motion exercises, or accompanying patients on short walks to promote mental and physical wellness.
Accurately document care notes, progress, and concerns in the digital charting system before the end of each shift.
