Requirements
- Comfortable and confident visiting patients in their homes across the Bemidji area.
- Dependable transportation to provide timely and reliable care to clients.
Responsibilities
- Visit clients in their homes to provide essential support that empowers them to maintain their independence.
- Assist clients with everyday activities, including mobility support and personal hygiene, ensuring they live in a clean and safe environment.
- Forge genuine connections with clients and collaborate closely with Nurse Case Managers to address and report any health concerns.
