Requirements

A genuine desire to help and care for others
Ability to provide compassionate, respectful, and dependable care
Comfort working independently in clients’ homes
Reliable transportation to travel between client visits
Ability to communicate effectively with clients and the care team
Ability to perform the physical requirements of the position

Responsibilities

Help clients remain in their homes and maintain as much independence as possible.
Provide assistance with personal care, mobility, hygiene, light housekeeping, and other activities of daily living.
Get to know your clients and become a trusted, familiar part of their support system.
Observe and report changes in a client's condition or needs to the appropriate member of the care team.
Collaborate with Nurse Case Managers and other members of the care team to provide quality, compassionate care.
