Requirements
- Be comfortable and confident visiting patients in their homes across the Park Rapids and surrounding areas.
- Have dependable transportation to provide timely and reliable care to our valued clients.
Responsibilities
- Visit clients in their homes where you'll provide essential support that empowers them to maintain their independence.
- Assist clients with everyday activities, from mobility support to personal hygiene, ensuring they live in a clean and safe environment.
- Forge genuine connections with your clients, becoming a trusted part of their lives.
- Collaborate closely with our Nurse Case Managers to promptly address and report any health concerns.
