About The Position

Always Best Care Senior Services is seeking compassionate Certified Home Health Aides (HHA) for flexible Per Diem/Per Visit opportunities throughout Solano County. This role involves providing one-on-one care in patients' homes, assisting them to remain safe, comfortable, and independent. The Home Health Aide works under the supervision of a Nurse Case Manager, Registered Nurse, and Director of Patient Care Services, following an individualized Plan of Care. This position offers a flexible schedule and the chance to make a meaningful impact on patients' lives.

Requirements

  • Current California Certified Home Health Aide (HHA)
  • Successful completion of a CMS-approved Home Health Aide Training and Competency Evaluation Program OR Successful completion of a State-approved Nurse Aide Training Program meeting federal requirements
  • State licensure meeting CMS Home Health Conditions of Participation
  • Current CPR Certification
  • Valid California Driver's License
  • Reliable, insured vehicle
  • Reliable transportation to patient visits
  • Ability to read, write, and communicate effectively in English
  • Successful completion of required background screening
  • Ability to work independently while following the patient's Plan of Care
  • Professional, dependable, and compassionate approach to patient care

Responsibilities

  • Provide personal care services according to each patient's individualized Plan of Care (POC)
  • Assist patients with Activities of Daily Living (ADLs)
  • Assist patients with Instrumental Activities of Daily Living (IADLs)
  • Assist with bathing, dressing, grooming, toileting, and personal hygiene
  • Assist patients with ambulation and mobility
  • Maintain a clean, safe, and healthy home environment
  • Prepare nutritious meals when directed in the patient's Plan of Care
  • Assist with treatments delegated by the supervising Registered Nurse
  • Assist therapy personnel with rehabilitative activities as assigned
  • Provide medication reminders for medications ordinarily self-administered
  • Obtain and document vital signs
  • Encourage patients to remain as independent as possible
  • Provide emotional support to patients and family members
  • Complete accurate and timely documentation for every patient visit
  • Immediately report changes in the patient's condition or home environment to the supervising Registered Nurse
  • Participate as a member of the interdisciplinary care team
  • Attend required in-service education and training
  • Coordinate patient visits with the clinical supervisor
  • Notify the agency promptly regarding absences or scheduling conflicts
  • Wear company identification while providing patient care
  • Perform other duties as assigned.
