The Home Health Aide provides care and comfort measures to patients in their homes as directed by care plan, interdisciplinary plan of care, and the provider's medical plan of treatment. This is a Part Time position serving the Lexington region. The aide provides specific aspects of care under the direction of the clinical leader, reviews the care plan and records visit notes, and documents care as well as significant findings. They report concerns or changes to leadership, assist patients with promoting functional ability and maintaining a comfortable home environment, and follow the established plan of care at all times. Responsibilities include assisting with or providing basic personal care and hygiene, bedside care in accordance with Home Care/Hospice policies and procedures, assisting patients with daily living, personal hygiene (including baths, shampoos, nail care), ambulation and prescribed exercise. They also assist patients with toileting, provide proper skin care to incontinent patients, and assist with oral or topical medications that the patient can normally self-administer as directed by the patient’s careplan. Measuring fluid intake and output as indicated in the care plan is also part of the role. The aide reviews the care plan and asks leaders any questions prior to service, records visit notes, and reports any changes in the patient’s physical condition, behavior, or appearance to the care team.
