About The Position

Home Care Network (HCN) is seeking dedicated and compassionate Home Health Aides (HHAs) to join our PRN (As Needed) Relief Pool. This role is ideal for individuals seeking flexible work arrangements, such as those with existing full-time or part-time jobs, semi-retired individuals, or those needing to supplement other healthcare positions. As a PRN HHA, you will provide essential personal care services to clients in their homes, following a plan of care established by a Registered Nurse. You will be responsible for delivering direct patient care, measuring vital signs, assisting with daily living activities, performing light housekeeping, and reporting any changes in patient condition. This position offers the potential for long-term employment and career advancement within HCN, with priority consideration for recurring schedules, part-time, and full-time opportunities.

Requirements

  • Active CNA or STNA certification + 2 or more years of paid experience
  • OR Active DSP Certification from Ohio DODD + 2 or more years of experience caring for persons with disabilities AND willingness to care for non-DODD clients.
  • OR +3 or more years of paid home health experience for HHA or PCA without certification.
  • Successful completion of HCN's written skills test and/or competency evaluation.
  • Meet/Completion of all training and/or certifications required by the state in which the home health aide services are being provided.
  • CPR and First Aide Certifications (active/current certs preferred but may also be obtained prior to hire date through HCN or accredited third party).
  • Ability to perform all typical physical tasks on a regular basis, and willingness to do so safely, including: Frequent: standing, walking, stopping, ending; Infrequent: squatting/kneeling, balancing; Pushing and handling weight/force: 1-20 lbs. frequently, 20-30 lbs. occasionally, and 30-50 lbs. infrequently; Lifting and carrying: 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently.
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
  • Strong track record of attendance, dependability, and employment retention with previous employers.
  • Good interpersonal skills and compassion for helping others live their best lives.
  • Demonstrated ability to read, write, and follow a written Plan of Care, incorporating attention to detail in following instructions, documenting notes, and communicating with our agency.
  • Professional character traits: reliable, honest beyond reproach, punctual, articulate, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, self-driven, problem-solver, compliant, and confidential.
  • Be at least eighteen (18) years of age.
  • Valid driver's license/ID and reliable transportation to/from client care locations.
  • Consent and ability to pass a State and/or Federal Background Check.
  • Must have a positive attitude, strong caregiving skills, and a compassionate mindset.
  • Must be comfortable working in a variety of home environments, including homes with pets, smoke odors, clutter, family members or roommates present, and other common residential conditions.
  • Must maintain flexibility to accept assignments based on client and business needs within their stated availability.
  • PRN schedules may include days, evenings, weekends, holidays, short-notice assignments, as well as varying shift lengths, schedules, and client assignments.
  • Accurate and timely documentation is a condition of employment.

  • Specialized certification or training program, such as DODD-DSP, Med Certification, EMR/EHR, etc.
  • EMR/EHR experience and proficiency (ContinuLink highly desirable).

Responsibilities

  • Provide direct patient care and support, following the established plan of care, to pediatric, adult, and geriatric patients.
  • Measure and record vital signs and other specified metrics as instructed.
  • Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
  • Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
  • Assist patients in taking medications under the direction and supervision of the Registered Nurse (opening containers or pill caddies).
  • Identify and report any abnormal findings or changes in the patient's condition.
  • Help with therapy exercises under the supervision of therapists (e.g., range of motion exercises, ambulation assistance).
  • Complete all required documentation accurately, thoroughly, and on time, including care notes, visit records, EVV requirements, and incident reporting.
