The Home Health Aide position contributes to effective patient outcomes by providing competent patient care assigned. The aide will perform personal care needs, laundry, light cleaning, and ensure patient safety. They will also take vital signs, provide skin care, change linens, and maintain patient confidentiality. The position requires the ability to work independently, maintain a professional schedule, cooperate with team members, and be comfortable with end-of-life care, providing a calm presence in the patient's home. Effective communication, participation in case conferencing, and accurate documentation are essential. The role also involves participating in quality improvement processes and adhering to all regulatory requirements, including annual inservices and OSHA standards.
