This is a part-time Home Health Aide position with Blue Ridge Home Health Aides, requiring 24 hours per week. The role involves providing competent patient care, assisting patients with activities of daily living (ADLs) to help them reach their functional status goals, and monitoring for changes in patient condition. The aide will also perform personal care needs, ensure patient safety, take vital signs, provide skin care, and change linens and clothing. The position requires the ability to work independently, maintain a professional schedule, cooperate with team members, and be comfortable with issues of death and dying, providing a calm presence. Effective communication skills are essential for interacting with patients, families, and healthcare team members. Documentation of patient care must adhere to agency, state, federal, and accrediting organization standards, while maintaining patient confidentiality. The role also involves participating in quality improvement processes, demonstrating cost-effective measures, assisting with problem identification, and ensuring patient satisfaction. Adherence to regulatory requirements, including annual inservice hours, hospice education hours, OSHA requirements, and state/HIPAA training, is mandatory.
