The Home Health Aide (HHA) – Hospice/Home Health position involves providing competent patient care and contributing to effective patient outcomes. The role requires assisting patients with ADLs (Activities of Daily Living) to help them reach their functional status goals, while maintaining safety. The HHA will note changes in patient condition, inform the case manager or RN/Supervisor, and document accordingly. Understanding the hospice philosophy and being comfortable providing specialized care to the terminally ill is essential. Responsibilities include performing personal care needs such as feeding, laundry, light cleaning of the patient's room, and washing. The role also involves meeting patient safety needs by using equipment safely and properly, taking vital signs (temperature, pulse, respiration, blood pressure) as directed, and providing skin care and repositioning patients to prevent breakdown. Changing patient bed linens and clothing as requested by the patient and family is also part of the duties. The position requires the ability to work independently, attend to the work schedule professionally, cooperate with team members, and be secure with issues of death and dying, maintaining a calm demeanor. Effective communication skills are crucial for interacting with all customers, participating in case conferencing and IDGs (Interdisciplinary Group Meetings) to achieve patient outcomes, preparing documentation according to agency, state, federal, and accrediting organization standards, and maintaining patient confidentiality in compliance with HIPPA/Privacy acts. Active participation in the CQI (Continuous Quality Improvement) process, demonstrating cost-effective measures, assisting with problem identification and solutions, participating in quality improvement projects, and demonstrating measures to ensure patient satisfaction are also key aspects of the role. Adherence to regulatory requirements, including maintaining annual inservice hours, Hospice education hours, OSHA requirements, cooperating with supervisory visits, providing services in accordance with regulatory rules and standards, and meeting state and HIPPA training requirements are mandatory. Professionalism and cooperation with all associates, flexibility in scheduling to support the department and other service offices, identifying personal developmental needs, demonstrating self-direction in maintaining skills, and commitment to team, office, company, and organizational goals are expected.
