About The Position

Emplify Health is seeking a Home Health Aide (CNA) to join its compassionate Hospice team. The role provides essential care and support to patients and families during end-of-life transitions, helping ensure comfort and dignity. The position is 0.7 FTE, working 56 hours bi-weekly in 8-hour day shifts, with participation in an on-call rotation every fourth weekend and one holiday per year.

Requirements

• High school diploma or equivalent.
• Completion of a Home Health Aide/CNA program meeting state and federal requirements for placement on the Wisconsin and Minnesota State Home Health Aide Registry upon hire.
• At least 1 year of CNA experience in acute care, a nursing home, or a home health agency.
• Wisconsin Registered Certified Nursing Assistant credential upon hire.
• Minnesota Registered Certified Nursing Assistant credential within 90 days of hire.
• BLS CPR certification from the American Heart Association or American Red Cross, including hands-on learning.
• Valid driver’s license.

Responsibilities
