About The Position

Home Care Network, Inc. is seeking experienced, passionate, and professional Home Health Aides (including HHA, STNA, CNA, PCA, or DoDD DSP) to support new clients in Southern Ohio. The role involves providing daily care, skill development, health monitoring, and advocacy to clients across Brown County, Ohio, and surrounding areas. This is a long-term employment opportunity with part-time and full-time positions available, designed to fit individual availability and preferences, offering consistent, predictable weekly schedules.

Requirements

  • Minimum of 1 year paid in-home care or caregiving experience
  • Actively-certified DSPs, STNAs and CNAs w/ +1 year experience are highly desired (or HHA/PCA with 2+ years paid experience)
  • DSPs also require a valid DoDD DSP certificate within the past 12 months and familiarity with Ohio Board of DoDD rules.
  • Successful completion of all HCN skills tests and competency evaluations.
  • Willingness and timely compliance to maintain all ongoing Training & Certification Requirements
  • Active CPR + FA certification (+ proof/documentation of both readily available)
  • 18yo + active Driver's License / State ID
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).
  • Consent + Ability to pass a State & Federal background check (including fingerprints)
  • Willing and able to regularly perform all physical demands of home care (including lifting/carrying up to 30–50 lbs. occasionally).
  • Ability to build trust with individuals; Strong communication skills (verbal & written); Patience, empathy, and compassion.
  • Ability to implement behavior support plans; De-escalation and problem-solving skills; Ability to promote independence rather than dependence.
  • Recognize changes in health status; Follow care plans and medical protocols; Understand and apply infection control and safety procedures.
  • Ability to complete daily service documentation, incident reports, and EVV (Electronic Visit Verification) entries.
  • Possess above-basic computer and mobile device proficiency.
  • Must adhere to Individual rights protections (Ohio Bill of Rights for individuals with DD), Confidentiality (HIPAA compliance), and Mandatory reporting laws (abuse, neglect, exploitation).
  • Must maintain professionalism with individuals, families, and care teams.
  • Either Active CNA or STNA certification + 1 or more years of paid experience
  • Or +2 or more years of paid home health experience for HHA or PCA without certification.
  • Active DSP Certification from Ohio DODD + 1 or more years of experience caring for persons with disabilities.

Responsibilities

  • Deliver direct patient care and support, following the established plan of care.
  • Measure and record vital signs and other specified metrics as instructed.
  • Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
  • Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
  • Assist patients in taking medications under the direction and supervision of the Registered Nurse (opening containers or pill caddies).
  • Identify and report any abnormal findings or changes in the patient's condition.
  • Help with therapy exercises under supervision of therapists (e.g., range of motion exercises, ambulation assistance).
  • Know and follow infection control policies including proper hand washing techniques.
  • Report any observed or reported changes in the client's condition and/or needs to the Registered Nurse.
  • Document care provided and complete all forms required for the client's records.
  • Promote personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations.
  • Demonstrate safe practice in the use of equipment.
  • Communicate effectively with all members of the interdisciplinary team.
  • Treat patients and families with respect and maintain confidentiality.
  • Accept and fulfill assignments with the agency.
  • Participate in required in-service programs.
  • Perform simple procedures as an extension of therapy services under the direction and supervision of the therapist.
