About The Position

Home Care Network (HCN) is rapidly expanding in Northeast Ohio, driven by hospital and referral partner needs for immediate care initiation (within 24-48 hours). To meet these demands, HCN is establishing a Ready to Work New Employee Pool of fully onboarded caregivers. This pool offers earlier access to cases, the ability to start working and earning quickly, and priority access to additional hours and new cases within your service area. This role is ideal for reliable, compassionate, and experienced caregivers who excel in self-driven, one-on-one care environments. The position offers priority access to cases matching personal and professional preferences, weekly pay, and strong support from a mission-driven team. As a member of the Ready to Work New Employee Pool, you will complete all onboarding upfront to become "case-ready" and available to start new case assignments within 24-48 hours of being matched. You will quickly transition to your preferred schedule type, starting part-time and building up to your desired number of hours per week (part-time or full-time). Cases will be aligned with your location, travel radius, experience, and availability, with priority access to new cases in this high-demand area. HCN offers weekly pay and strong support and communication from care coordination teams. This role provides the same culture, pay, and support as standard caregiving positions but with faster case placement and more opportunities to build an ideal schedule. HCN guarantees pay for the first 60 days at the full number of hours committed per week, regardless of actual hours worked during the ramp-up period, for participants committing to at least one year of employment with HCN and maintaining reliable performance. This model helps secure more cases, providing earlier access to work opportunities in competitive areas and predictable income during ramp-up.

Requirements

  • CHHA, CNA, STNA or DSP certification (state-specific) -OR- a minimum 2 years paid home health experience (in lieu of certification).
  • Active CPR and First Aide Certifications (or willingness to obtain via 3rd party prior to hire date).
  • Ability to start care within 24–48 hours once matched.
  • Commitment to complete all HCN onboarding requirements within 5–10 days, including: Required HR, State & Federal paperwork + documentation uploads, HCN competency evaluation, skills tests, and policy, procedure & EMR/EHR documentation training, In-person fingerprinting.
  • Consent + successfully pass all State and/or Federal Background Checks and Reference Checks.
  • Consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius.
  • Willing and able to regularly perform all physical demands of home care (including lifting/carrying up to 30–50 lbs. occasionally).
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
  • Must be at lease eighteen (18) years of age.
  • Strong attendance history, dependable employment record, and stable employment retention.
  • Clear communication skills, accurate documentation, and ability to follow a Plan of Care.
  • Desire for long‑term employment & stable client-care mindset (2+ years preferred minimum).
  • Compassionate, respectful, accountable, detail‑oriented.
  • Demonstrated ability to read, write, and follow a written Plan of Care, incorporating attention to detail in following instructions, documenting notes, and communicating with our agency.
  • Willingness to consent to a background check, including a criminal record check, and an employment and education verification.

Nice To Haves

  • Up to 45- or 60-minute client travel radius (from your home) is highly desired.
  • 3-5 years of paid experience in home health / agency.
  • Specialized certifications (DODD-DSP, Med Certification, dementia, etc.).
  • EMR/EHR experience and proficiency (ContinuLink is a plus).
  • Willingness to pick up additional hours as cases become available and/or provide client coverage for co-workers.

Responsibilities
