Requirements
- Home Health Aide (HHA) License to practice in California - Required
- Experience in Home Health or Hospice care - 1 Year Required
- Current CA Driver’s License
- Able to travel within a 15 - 25 mile radius on a regular basis
Responsibilities
- Evaluate patients' health
- Create and implement care plans
- Track patient progress to maintain the highest standards of care
- Provide support both remotely and independently in the field
