About The Position

Emplify Health is seeking a Home Health Aide (CNA) to join their compassionate Hospice team. In this role, you will provide essential care and support to patients and families during end-of-life transitions, ensuring comfort and dignity every step of the way. Emplify Health is a partnership of Bellin Health and Gundersen Health System, a leading not-for-profit, patient-centered healthcare network based in Green Bay and La Crosse, Wisconsin. With 11 hospitals and over 100 clinics, they serve 67 cities across Wisconsin, Iowa, Minnesota, and Michigan's Upper Peninsula.

Requirements

• High School Diploma or equivalent
• Completion of a Home Health Aide/CNA program meeting state and federal requirements for placement on the WI & MN State Home Health Aide Registry upon hire
• Minimum of 1 year CNA experience in acute care, nursing home, or home health agency
• WI Registered Certified Nursing Assistant upon hire
• MN Registered Certified Nursing Assistant within 90 days of hire
• BLS CPR certification from American Heart Association or American Red Cross with hands-on learning
• Valid driver’s license

Responsibilities

• Work 0.8 FTE (64 hours bi-weekly)
• Provide care during 8-hour day shifts
• Participate in on-call rotation every 4th weekend and 1 holiday per year
