Hospice Home Health Aide

Delivering Solutions that Matter: Provide compassionate, hands-on personal care to hospice patients, including bathing, grooming, dressing, toileting, and mobility assistance. Monitor and report changes in patients' physical condition, comfort, behavior, and needs to the hospice nurse or care team. Assist with maintaining a safe, clean, and comfortable home environment for patients and their families. Offer emotional support and companionship while respecting patient dignity, preferences, and end-of-life care goals.
Requirements
- Current, active MA CNA certification
- Minimum 1-year CNA experience
- Must have a valid driver's license
- Must have reliable transportation and the ability to travel to patient homes or hospice facilities.
- Ability to communicate fluently in English – both verbally and written
Nice To Haves
- Prior experience in home-care, hospice, rehabilitation, or long-term care setting preferred
