Requirements

  • Must meet educational/experience requirements outlined in the laws of licensure states.
  • Certified/licensed and in good standing within state currently working in according to clinical type.
  • Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
  • Possesses and maintains current CPR certification.
  • Possess and maintains a valid driver’s license.

Responsibilities

  • Provide compassionate personal care and assistance with activities of daily living, including bathing, grooming, dressing, oral hygiene, toileting, skin care, and maintaining a clean and orderly environment.
  • Prepare and assist with meals and feeding as needed, while supporting patients’ nutritional and hydration needs.
  • Assist patients with ambulation, exercise, range-of-motion activities, and other care activities as directed by the Plan of Care and within the HHA’s training and scope.
  • Monitor and record vital signs and other patient observations as ordered and within established training requirements.
  • Assist patients with medication self-administration and other routine care needs as directed by the supervising nurse or therapist.
  • Observe patients for changes in condition and promptly report concerns or significant changes to the supervising nurse and case manager.
  • Maintain a safe, respectful, and supportive care environment while completing accurate documentation and following LORIAN’s care standards, procedures, and professional expectations.
