Home Care Network is expanding and actively seeking experienced Home Health Aides, including CNA, STNA, DODD-Certified DSP, HHA, CHHA, PCA, etc., interested in providing one-on-one care in the home setting. Part-time positions offer the ability to accept one or multiple clients, specify available days & hours, and have custom travel parameters. PRN opportunities are available for as-needed, on-call, or back-up relief client visits, coverage for vacations and leaves, and supplemental HHA assignments. This role is best suited for individuals who desire flexibility and can accommodate a schedule that changes based on client needs. PRN work is based upon agency and client needs and is never guaranteed. Hours vary significantly from week to week for PRN positions.

Requirements

  • Minimum of 1-2 years paid in-home care or caregiving experience (agency experience desirable but not required)
  • STNAs and CNAs highly desired (or HHA/PCA with 2+ years consistent experience)
  • Active DSP Certification from Ohio DODD with 1+ year of experience caring for persons with disabilities AND willingness to care for non-DODD clients.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type.
  • Desire for long-term employment with our agency (no temp or short-term employment at this time)
  • Successful completion of HCN's written skills test and/or competency evaluation (Requires approx. 3-5 hours virtual testing & training prior to hire).
  • Active CPR + FA certification and documentation
  • Consent + Ability to pass a State & Federal background check (including fingerprints)
  • 18yo + active Driver's License / State ID
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius
  • Deep commitment to quality patient care
  • Excellent documentation habits and Plan of Care adherence
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
  • Ability to perform all typical physical tasks on a regular basis, and willingness to do so safely.

Nice To Haves

  • Additional Medication Certifications, Hoyer lift experience, specific disease-state care experience, etc., are highly desirable but not required.
  • Willingness to cover clients up to a 45- or 60-minute radius is highly preferred, especially if additional hours / additional clients are desired (not required) for PT Positions.

Responsibilities

  • Deliver direct patient care and support, following the established plan of care, to pediatric, adult, and geriatric patients.
  • Measure and record vital signs and other specified metrics as instructed.
  • Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
  • Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
  • Assist patients in taking medications under the direction and supervision of the Registered Nurse. This includes opening a container or pill caddy & handing it to the patient but does not include administration of the medications.
  • Identify and report any abnormal findings or changes in the patient's condition.
  • Help with therapy exercises under supervision of our therapists (e.g., range of motion exercises, ambulation assistance).
