Home Health Admitting Registered Nurse RN
About The Position
We are seeking a compassionate and experienced Registered Nurse (RN) to complete Start of Care (SOC) admissions for patients receiving home health services. The Admitting RN performs comprehensive assessments, completes OASIS documentation, develops individualized plans of care, and collaborates with physicians and the interdisciplinary team to ensure a smooth transition to home health care. Requirements
- Current Florida RN license.
- Minimum of one year of nursing experience; home health experience preferred.
- OASIS experience preferred.
- Current BLS certification.
- Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities
- Complete Start of Care (SOC) and admission assessments.
- Accurately complete OASIS documentation.
- Develop and coordinate patient care plans.
- Educate patients and caregivers on medications, disease management, and home safety.
- Communicate with physicians and the care team to coordinate services.
- Maintain timely, accurate documentation and ensure compliance with agency and regulatory standards.
