About The Position

Accurate Home Care is hiring a Registered Nurse for its Home Health service line. The nurse provides one-to-one care and serves patients within an approximate 60-mile radius of the Otsego office, with efforts made to keep territories condensed and close to home. The position focuses on admission and visit nursing, including patient assessment, care planning, coordination, monitoring, education, and treatment. Requirements

Be licensed as a Registered Nurse (RN) in Minnesota.
Maintain Basic Life Support (BLS) CPR certification for healthcare providers through the American Heart Association or Red Cross, renewed at recommended frequencies.
Be able to lift, turn, position, and transfer up to 50 pounds.
Be fluent in English.
Have a valid U.S. driver’s license with minimal moving violations and access to a reliable personal vehicle.

Responsibilities

Provide appropriate patient care through admission assessments, care planning, goal setting, evaluation, coordination of care, and emergency and safety planning.
Complete OASIS assessments at prescribed time points, including Admission/Start of Care.
Identify potential referrals to other disciplines for services not yet ordered by the physician.
Collaborate with clients, families, physicians, and other members of the patient care team.
Assess and monitor patients and maintain current documentation in the clinical record.
Report patient concerns to the Director of Home Health.
Instruct patients and families on illness, treatment regimens, and proper health maintenance.
Administer medications and treatments as prescribed by the physician, within scope of practice and in accordance with company policies and procedures.
