About The Position

The first visit shapes everything. Patients are often leaving the hospital, feeling overwhelmed, and unsure of what comes next. They're looking for someone who can confidently assess their needs, answer their questions, and create a plan they can trust. As an Admissions RN, you'll serve as the clinical leader who ensures every patient begins home health with confidence, clarity, and the right plan for success. This role is different because you become the trusted clinical expert who launches every episode of care, focusing on assessments, education, and care planning instead of routine follow-up visits. You will work independently while managing your own schedule and territory, build strong relationships with referral partners, patients, and families from day one, and use your experience to make immediate, high-level clinical decisions.

Requirements

  • Active Minnesota Registered Nurse (RN) license.
  • Strong clinical assessment and critical thinking skills.
  • Exceptional communication and patient education abilities.
  • Ability to work independently and manage a field-based schedule.
  • Reliable transportation.

Nice To Haves

  • Home health and OASIS experience strongly preferred.

Responsibilities

  • Complete comprehensive Start of Care (SOC) admissions throughout the Twin Cities Metro.
  • Perform detailed nursing assessments and complete accurate OASIS documentation.
  • Develop individualized plans of care that set patients up for long-term success.
  • Educate patients and families on diagnoses, medications, services, and expectations.
  • Coordinate seamlessly with physicians, case managers, therapists, and the interdisciplinary care team.
  • Ensure smooth, safe transitions from hospitals, skilled nursing facilities, and clinics into home health.
