About The Position

This position is responsible for the administrative and clinical functions of the Home Health services provided through University of Iowa Health Care Medical Center Downtown. This includes ensuring services are delivered effectively and efficiently by qualified staff. This position must have experience in health care operations.

Requirements

• Master's degree or an equivalent combination of education and experience is required.
• 3-5 years of supervisory or administrative experience within home health care or a related health care program is required.
• Demonstrate skill in leadership/management is required.
• Excellent written and verbal communication skills are required.

Responsibilities

• Monitors overall staffing and shift scheduling.
• Hire, develop, and manage the performance of staff in a departmental unit or functional division.
• Provide clinical education to develop staff competency.
• Ensure staff compliances are applicable, complete and current.
• Assist with and administer hospital approved contracts with outside individuals or agencies whose services are required for home health programs.
• Assure the on-call availability of appropriate and competent staff during evenings, nights, and weekends.
• Performs periodic performance evaluations of supervised staff as required by hospital policy.
• Foster a culture of accountability, professional development, staff engagement, workforce well-being, and retention.
• Prepare, monitor, and analyze the approved annual expense budget, and coordinates cost containment efforts in Home Health.
• Manage financial and productivity data and monitor the budget to meet goals.
• Resolve budget discrepancies.
• Prepare and/or assist in capital budgeting and equipment selection/replacement.
• Provide overall administration of home health services
• Maintain the program in compliance with applicable laws, regulations and accreditation standards, and in keeping with hospital and medical staff policies.
• Assume primary responsibility for establishing and administering a quality improvement program in cooperation with nursing administration and the hospital quality improvement coordinator.
• Compile and analyze infection control and prevention statistics and develop annual Home Health Infection Control Plan in coordination with the hospital infection control committee and infection preventionist.
• Oversee the Home Health Emergency Preparedness plans and activities.
• Serve as a member of all home health care committees.
• Prepare statistical and clinical reports required by the hospital for certification or other purposes
• Monitor Home Health patient satisfactions surveys and develop and monitor plans to enhance patient satisfaction.
• Prepare first drafts of new or revised policies or procedures and seek their approval.
• Frequently review each patient's clinical record to assure that documentation by all individuals serving the patient is clear, concise, goal-oriented and timely.
• Maintain the integrity of active clinical records and forwards records of discharged patients to the hospital's centralized medical records department.
• Coordinates and reports to the Home Health Governing Board and acts as a liaison between the Governing Board and Home Health personnel.
• Ensure mechanisms are in place to monitor record quality, documentation compliance, and clinical outcomes.
• Participate in and provide leadership support to governance, quality, and operational committees as appropriate.
• Monitor agency performance related to CMS quality measures, HHVBP performance, patient outcomes, hospitalization rates, and patient experience metrics.
• Utilize quality data to drive clinical improvement initiatives and operational decision-making.
• Provide leadership for technology adoption, clinical documentation systems, data analytics, and workflow optimization.
• Maintains agency compliance with all Medicare Conditions of Participation, accreditation standards, and organizational policies.
• Serves as the accountable leader for agency survey readiness and response to regulatory and accreditation reviews.
• Identify needs, trends, and forecasts to provide quality patient care.
• Select, schedule, and evaluate the services of directly supervised staff.
• Appoint Home Health Clinical Manager and ensure Clinical Manager is available during all operating hours.
• Coordinate the clinical activities of all staff, whether hospital-based or contracted, who are actively engaged in providing home health services.
• Build and maintain collaborative relationships with referral sources, hospitals, physicians, post-acute providers, and community agencies.
• Provide leadership for technology adoption, clinical documentation systems, data analytics, and workflow optimization.
• Ensures administrative and operational functions are organized, managed, and monitored to support safe, effective, and compliant patient care delivery.
• Develop action plans for the unit that meet strategic goals; provide progress reports to the Associate Chief Nursing Officer.
• Participate in and serve as a contact for facility projects including those related to services, teaching, administrative and research areas as directed.
• Develop and implement strategies to support service growth, market expansion, and alignment with health system strategic priorities.
• Evaluate new service opportunities and community needs to support population health initiatives.
• Identify emerging trends and recommend operational and clinical innovations.
• Advocate for needs of the unit with healthcare leaders; may collaborate with the Associate Chief Nursing Officer of the unit regarding program development, process improvements, and equipment budgets.
• Meet individually or collectively with staff nurses, physicians, or others in order to explain the range of home health services available.
• Convene and chair required multi-professional patient staffing conferences and assures that these meetings are goal-oriented and documented.
• Ensures accuracy of public information materials and activities in conjunction with community relations.
• Provide clinical education to develop staff competency.
• Oversee clinical education activities and agreements with other educational institutions.
