Requirements

  • Bachelor's Degree in Nursing
  • Current state license as a Registered Nurse
  • Minimum 2 years of experience in nursing
  • Minimum 2 years of experience in home health
  • Proven experience with regulatory and/or leadership experience
  • Demonstrated knowledge of the appropriate skills for communicating with all ages, especially the geriatric population

Responsibilities

  • Responsible for the overall direction of the Home Health services.
  • Responsible for the employment of qualified Home Health personnel.
  • Responsible for the provision of home health services, directly, and the delegation to and coordination of hospice personnel evaluations.
  • Responsible for establishing standards of care to comply with federal and state regulations and guidelines.
  • Establishes, implements, and evaluates goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization and philosophy.
  • Provides expert guidance and assistance in all matters relating to personnel and patient care services.
  • Responsible for providing oversight and helping to manage patient and personnel assignments to ensure optimal management of the complex and interdisciplinary care of home health patients.
  • Responsible for ensuring coordination of care for patients through development and implementation of policies and procedures, personnel education and in-services, and assisting in care plan development with clinical staff.
  • Responsible for ensuring coordination of referrals for patients.
  • Responsible for ensuring the assessment of patient needs and ensuring the plan of care is developed, implemented, and updated.
  • Assists in the development and interpretation of the philosophy and objectives of the agency.
