Requirements
- Bachelor's Degree in Nursing
- Current state license as a Registered Nurse
- Minimum 2 years of experience in nursing
- Minimum 2 years of experience in home health
- Proven experience with regulatory and/or leadership experience
- Demonstrated knowledge of the appropriate skills for communicating with all ages, especially the geriatric population
Responsibilities
- Responsible for the overall direction of the Home Health services.
- Responsible for the employment of qualified Home Health personnel.
- Responsible for the provision of home health services, directly, and the delegation to and coordination of hospice personnel evaluations.
- Responsible for establishing standards of care to comply with federal and state regulations and guidelines.
- Establishes, implements, and evaluates goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization and philosophy.
- Provides expert guidance and assistance in all matters relating to personnel and patient care services.
- Responsible for providing oversight and helping to manage patient and personnel assignments to ensure optimal management of the complex and interdisciplinary care of home health patients.
- Responsible for ensuring coordination of care for patients through development and implementation of policies and procedures, personnel education and in-services, and assisting in care plan development with clinical staff.
- Responsible for ensuring coordination of referrals for patients.
- Responsible for ensuring the assessment of patient needs and ensuring the plan of care is developed, implemented, and updated.
- Assists in the development and interpretation of the philosophy and objectives of the agency.
