About The Position

The Home Community Support Associate provides assistance with activities of daily living, including eating, meal planning, bathing, dressing, and hygiene or toileting. The role also supports eligible individuals with related light housekeeping, access to community resources, shopping, and appointments. The associate follows the Activity Plan and/or family instructions for personal care, uses safe transportation procedures, documents services and contacts, and follows incident-reporting, safety, and workers' compensation guidelines. The schedule may be flexible based on the family's needs.

Requirements

High school diploma or equivalent.
Must be at least 21 years of age.
Must have a valid driver's license.

Responsibilities

Assist with activities of daily living, including eating, meal planning, bathing, dressing, and hygiene or toileting.
Provide light housekeeping directly related to the eligible individual's health and well-being, such as bed-making, dusting, and vacuuming.
Assist with accessing community resources, shopping, and appointments.
Complete and submit Daily Activity Logs and Contact Summaries accurately and in a timely manner.
Adhere to safe transportation procedures.
Follow appropriate protocols for completing I/A and Serious Incident reports.
Work a flexible schedule based on the family's needs.
Attend trainings and in-services as requested and provide requested personnel update information in a timely manner.
Follow the Activity Plan and/or family instructions regarding the eligible individual's personal care.
Keep the supervisor informed of pertinent information relevant to service provision.
Follow safety policies and workers' compensation program guidelines.
Perform other functions and tasks as assigned.
